Hygienist Completes the following assessments

Dental Hygiene Clinical & Handoff Protocol
Hygienist: Required Assessments
1. Medical & Dental History – Update at every visit; verify medications and conditions relevant to dental care. Take and record vital signs (pulse, blood pressure) each appointment to identify undiagnosed or uncontrolled hypertension and adjust care as needed.
2. Radiographs & Intraoral Photos – Order radiographs based on individual caries/periodontal risk and clinical indications (not fixed intervals), following the ADA/FDA selection guidelines; capture IO photos for patient education and documentation.
3. Oral Cancer Screening – Perform visual/tactile head-and-neck/oral soft-tissue exam; use adjuncts only as triage tools when indicated.
4. DMFT & CAMBRA – Complete a caries risk assessment and tie preventive/restorative plans to risk level (e.g., fluoride, sealants, recall interval, diet counseling).
5. Periodontal Charting – Full charting per office policy; classify Stage & Grade and link to a treatment plan (e.g., D4346, SRP, maintenance interval) with systemic risk modifiers (e.g., smoking, HbA1c) considered.

Hygienist Communicates Patient is Ready

Hygienist → Doctor: “Patient Ready” Notification (Radio Headset)
• If not periodontally/medically healthy: “Patient in Op #__ ready for exam to confirm hygiene findings before I proceed with cleaning.”
• If healthy: “Patient in Op #__ ready for exam.”
Schedule ideal exam windows during the morning huddle so doctors can step in efficiently and minimize operatory idle time.

Route Sheet is Used

Route Sheet + Verbal Front Desk Handoff
Before dismissal, complete and verbally relay:
• Hygiene services completed (tooth/quadrant specificity) & exam type
• Next hygiene visit scheduled (interval tied to risk/perio status)
• Additional hygiene/restorative appointments needed
• Prescriptions/products dispensed
• Referrals placed
• Unaccepted treatment requiring follow-up
• Family members due/recall opportunities
(Verbal handoff prevents missed steps and aligns the clinical → admin workflow.)

Available On Operatory Monitor:

What Must Be Visible on the Operatory Monitor
• Intraoral photos highlighting the chief complaint / suspected lesions
• Current radiographs (selected per ADA/FDA guidance)
• Recent scan (e.g., iTero) for visualization and case acceptance
(IO images strongly support patient understanding and acceptance.)

Other Notes:

Morning Huddle Responsibilities (Hygienist)
• Flag patients due for perio charting or likely exam code changes
• Identify best doctor exam times for hygiene columns and mark them
• Confirm imaging needs (radiographs, IO photos, scans) and room readiness
• Align on same-day start opportunities (e.g., D4346/SRP blocks)
(Morning huddles consistently improve daily flow and outcomes.)

Documentation Essentials
• Record vitals and any action taken (e.g., elevated BP protocol/MD referral)
• Chart Perio Stage/Grade, CAMBRA risk category, and the linked plan
• Note patient education delivered (showing images/scan), and acceptance/declines
• Use standardized terms/codes consistent with ADA/AAP guidance
